April 16-22 National Health Care Decisions Day (NHDD)

Now in its 11th year, National Healthcare Decisions Day exists to inspire, educate and empower the public and providers about the importance of advance care planning. NHDD has expanded their celebration to a week nationally.  Suggestions for ways to implement NHDD activities in your community and at your facility are on the NHDD website.

Also, the nonprofit organization Honoring Choices Massachusetts offers a free, online “Getting Started Toolkit” that walks you through easy steps to choose and designate a healthcare agent, and fill out a basic healthcare proxy document. The forms and information can be reviewed and downloaded here.
